2. We Do Not Provide Legal Advice
No information on this website — including content on our resource pages, guides, articles, blog posts, interactive tools, eligibility survey results, chatbot conversations, or any other communication from us — constitutes legal advice or creates an attorney-client relationship between you and My Diabetes Benefits, CaseHelix, LLC, or any of our employees, contractors, or affiliates.
Our eligibility survey provides a preliminary assessment only. Your pre-qualification score is based on general criteria and the information you provide. It is not a legal opinion about the merits of your disability claim. Only a licensed attorney who has reviewed the specific facts of your case can provide legal advice.
Our attorney matching service is not a legal referral. When we connect you with a disability attorney or representative, we are facilitating an introduction — not recommending or endorsing that specific attorney. Any attorney-client relationship is formed directly between you and the attorney, governed by a separate engagement agreement. We do not control, direct, or supervise the attorney's work on your case.

13. Dispute Resolution
Informal Resolution First: Before filing any formal legal proceeding, you agree to contact us at legal@mdbenefits.org to attempt to resolve any dispute informally. We will try to resolve the dispute within 30 days.
Binding Arbitration: If we cannot resolve the dispute informally, you and My Diabetes Benefits agree that any dispute, claim, or controversy arising out of or relating to these Terms or your use of our Services shall be resolved through binding individual arbitration administered by the American Arbitration Association (AAA) under its Consumer Arbitration Rules. Arbitration shall take place in the county of your residence or, at your election, by telephone or video conference.
Class Action Waiver: You agree that any dispute resolution proceedings will be conducted only on an individual basis, and not in a class, consolidated, or representative action. If for any reason a claim proceeds in court rather than in arbitration, you waive any right to a jury trial.
Exceptions: Either party may bring a claim in small claims court if it qualifies. Nothing in this section prevents you from reporting a complaint to a federal, state, or local government agency, including the Federal Trade Commission (FTC) or your state attorney general.
Opt-Out Right: You may opt out of this arbitration provision by sending written notice to legal@mdbenefits.org within 30 days of first using our Services. Your notice must include your name, address, and a clear statement that you wish to opt out of arbitration.
14. Governing Law
These Terms are governed by and construed in accordance with the laws of the State of [State], without regard to its conflict of law principles.
